CLOSURE AT LUNCHTIME – From Monday 23rd we will be closed between 1 – 2pm daily. For URGENT acute medical attention please phone 91515215.
We are moving to Telephone triage of Appointment requests. If your requirement is NON URGENT please avoid making contact. The reception staff may require more detail from you.
Repeat Prescriptions orders are now going to Nominated Chemists – Please ensure you name a chemist for these to go to. These will now take 96 hours to process. Repeat line is 91515315.
Do NOT over order medications – this is a Health Board Directive – items MAY be rejected.
Acutes (medications that are NOT on repeat)– Can still be request via reception staff on the phone 91525215 and collected from reception.
Appointments – If you have an appointment and have SUSPICIONS of Coronavirus PLEASE DO NOT ATTEND – Please do attempt to cancel the appointment.
We are suspending online appointments until further notice.
We will NOT be issuing sick-lines for Coronavirus symptoms, certificates can be obtained via https://111.nhs.uk/isolation-note/

GP RETIREMENT - Dr Crawford has retired as a partner in the Practice as of 30 November. He has been in Bangor Health Centre for over 30 years and will be greatly missed by our team. We wish him the very best for his retirement and will be welcoming him as locum to support the Practice.
PRACTICE NAME - We have changed our Practice name to reflect Dr Crawford's retirement to CHURCH VIEW FAMILY PRACTICE. Our phone and repeat prescription lines will also change to our new name in due course.
FLU CLINICS - Our flu dates are now finished. If you have missed the flu vaccination and are aged 65+, or have a health condiditon eg, asthma, diabetes, copd, heart disease, please contact the Practice to book into the flu catch up clinics. Thank you.

Pharmacies Minor Ailments - new prescription items now available. Please contact your local Chemist for more details - only takes a minute to complete a form, 1 stop shop.
We operate a Do Not Attend Policy - multiple failures to attend appointments without prior notification will result in Patient removal from our Practice due to a breakdown of the patient/Doctor relationship.
Patient Survey 2018 - Thank you to all our Patients that took part in our Patient Satisfaction Survey. 94% of our patients are happy with our GP Practice and would recommend our Practice to someone moving to the area. A very positive patient satisfaction survey - for. We continue to show are commitment to reviewing our telephone access with the introduction of new telephone system, and we have changed our morning telephone access, we will continue to monitor patient demand. It would be helpful if patients could ease the pressure on phone lines by ONLY phoning for test results between 2 - 4pm, by ordering repeat mediations online, or on our answer phone 91515315 or by dropping a note into reception.
